In this form, you can book your free trial week at the Grey Wolf BJJ Academy.

The first day of your trial week must be on Monday (08:30 PM - 9:30 PM), Wednesday (7:00 PM - 8:00 PM), or Friday (7:00 PM - 8:00 PM).

Before submitting the request, remember to download and complete the forms:

Liability Waiver

• If you are an adult: adult-liability-waiver
• If you are a minor: minor-liability-waiver

Photo/Video Release Form

• If you are an adult: adult-photo-video-release
• If you are a minor: minor-photo-release

For your first class, remember to bring the completed forms, along with a copy of your ID and Tax Code.
